Legend has it that the Ujjaini Mahakali temple was established in 1813 by Suriti Appaiah, a member of the Indian military. Appaiah's journey to Ujjain was spurred by a mission to combat a cholera outbreak that had claimed thousands of lives. Upon seeking solace at the Mahakali temple in Ujjain, he made a solemn vow: if his entire platoon survived, he would install an idol of the goddess in Secunderabad.

The temple holds immense significance, drawing thousands of devotees each year to participate in the annual Secunderabad Ujjaini Mahankali Jatara, commonly known as Lashkar Bonalu, during the month of Ashadha.
