According to railway officials, the accident occurred at about 7 p.m. between Alamanda and Kantakapalli in Vizianagaram-Kothavalasa Railway Section of Waltair Division of East Coast Railway Zone. The accident occurred when the Visakhapatnam-Rayagada passenger train overshot the signal to hit Visakhapatnam-Palasa passenger train from rear while it was waiting for the signal.

The Railway Ministry officials said that 12841 Coromandel Express (Shalimar-Madras) passing through the main up line first met with an accident and dashed with the stationary goods train in the loop line at Bahanaga Bazar station. The official said that Coromandel Express at the time of the accident was travelling at its full speed across the Bahanaga Bazar station as it was not supposed to stop there.

Driving patiently in the mad-hell traffic, I reached the Western Express Highway at Goregaon when my phone started ringing non-stop, around 9.30 p.m. Muttering irately, I stopped to answer, it was a professional colleague, almost screaming: "There's some shoot-out in Colaba area... Many police vehicles rushing there... some localities are cordoned off..." and abruptly cut off. Half-guessing it to be some kind of a mafia gang-war, I nevertheless made some strategic calls to confirm it was more than that.
